to check your insurance benefits prior to making your appointment
Unlike other
fields of medical insurance, patients who see their eye doctor are
sometimes seen using vision benefits and sometimes seen using medical
insurance benefits. Some patients have both types of insurance and
others have only medical insurance. In general, if you are interested
in a check of your vision, complete eye examination, and glasses
or contacts, you will be seen using your vision insurance. On the
other hand, if your medical doctor has referred you for an examination
or you are having a problem with your eyes other than needing new
lenses or a routine checkup, you will usually be seen using your
medical insurance benefits for a medical eye examination.
Some patients,
particularly those with conditions such as diabetes or glaucoma,
are allowed a complete medical eye examination each year by their
health plan. This varies from one plan to the next, so we recommend
you call your plan and ask.
It is important
for you to know your benefits and obtain any necessary referrals
prior to your examination. To help you, we recommend you follow
these steps:
- Contact the
health plan's phone number on your insurance card.
- If this is
a medical visit, ask your health plan if a referral from you Primary
Care Physician (PCP) is necessary. If so, please contact your
PCP for a referral. Some PCPs will not issue referrals to their
patients after the visit, so it is important that you call first.
Do you have a co-pay?
- If this is
a routine vision exam, ask the health plan if you have vision
benefits.
- How
often are you eligible for a routine vision exam?
- Do you
have a co-pay?
- Do you
have benefits for glasses and contact lenses?
- How often
are you eligible for glasses and/or contact lenses?
- What
does your insurance pay toward frames, lenses and contact
lenses?

How
to make an appointment
Our goal is
to make your visit to EyeHealth Northwest as convenient and enjoyable
as possible. Please see our locations
page and determine the EyeHealth Northwest location most convenient
for you. Call the appointment number for your preferred clinic and
ask to make an appointment or request an appointment online.
Please let the appointment scheduler know if you are a new or existing
patient. If you would prefer a particular doctor, please let the
scheduler know.

What
to bring to your appointment
Please bring
your eyeglasses, contact lenses, and any low vision aids you use.
Your doctor will also ask you to provide a list of your allergies
and medications (with dosages). This includes any eye drops, over
the counter medications, or supplements used in the past six months.
Your will also be asked to complete a form detailing your past medical
history. In some cases, your doctor may feel it is important to
obtain a copy of your previous medical records.
In order to for EyeHealth Northwest to bill your insurance properly,
you will also need to bring your current insurance card with you.
If your visit requires a referral from your Primary Care Physician,
please bring the referral information with you as well.

If
your eyes will be dilated
If your examination
requires that your eyes be dilated, please BE SAFE! If you are uncomfortable
driving after having your eyes dilated, please arrange to have someone
drive you to and from your appointment.
Will you be
selecting new glasses? If your eyes will be dilated and you are
planning to purchase new glasses, please try to arrive early enough
to browse our Optical Shop and select your frames prior to having
your eyes dilated. You will be able to make a better selection when
you can see more clearly.
