General Insurance Information

The physicians of EyeHealth Northwest are providers for most major insurance companies. The following list is a sample of just some of the insurances we participate with. If you are unsure if EyeHealth Northwest participates with your insurance, please call your insurance company and ask if the physician is a provider.

Insurance Billing Information

As a courtesy for our patients, EyeHealth Northwest will bill primary and secondary insurance provided the information is given at the time of service.

Prior to your appointment, contact your insurance company for verification of benefits. If your insurance requires a referral for medical visits, it is your responsibility to contact your primary care doctor to obtain one.

When verifying vision benefits, ask how often the exam benefit is available and what type, if any, benefits for glasses or contact lenses are available. If vision coverage is not clear, your coverage may be through Vision Service Plan (VSP). The representative should be able to confirm this.

If your vision coverage is through VSP, you will be required to provide this information at the time of ordering your glasses or contact lenses. EyeHealth Northwest can not bill VSP after frames and lenses have been ordered.

Payment Policies

If payment is not received within 60 days from your insurance company, EyeHealth Northwest will require payment from you. Payments received from your insurance company after you have paid will be promptly refunded to you.

Full payment is required at the time of service for all co-pays, deductibles, glasses, contact lenses and services not covered by insurance.

A $10 handling fee is added to CO-pays not paid at the time of service.

If you are unable to pay in full at the time of service, a payment plan may be extended. Please talk with one of our Account Representatives at 503-344-5115 to arrange a payment plan. Payment plans will not be extended to patients who have failed to make timely payments in the past.

Accounts that are 60 days old are considered delinquent. A finance charge of $3.00 per month or interest of 1 ½ % per month (whichever is greater) will be added to cover the cost of additional handling.

We accept assignment on Medicare.

Checks returned for insufficient funds, closed accounts or other problems are subject to a $25 handling fee.

Accounts subject to collection activity may be charged a $25 handling fee.
