Insurance Billing Information
As a courtesy
for our patients, EyeHealth Northwest will bill primary and secondary
insurance provided the information is given at the time of service.
Prior to your
appointment, contact your insurance company for verification of
benefits. If your insurance requires a referral for medical visits,
it is your responsibility to contact your primary care doctor to
obtain one.
When verifying
vision benefits, ask how often the exam benefit is available and
what type, if any, benefits for glasses or contact lenses are available.
If vision coverage is not clear, your coverage may be through Vision
Service Plan (VSP). The representative should be able to confirm
this.
If your vision
coverage is through VSP, you will be required to provide this information
at the time of ordering your glasses or contact lenses. EyeHealth
Northwest can not bill VSP after frames and lenses have been ordered.
