Welcome to EyeHealth Northwest online contact lens ordering! We are very pleased to offer this new service for our patients. At this time we only accept orders from patients who have been seen by an EyeHealth Northwest doctor.

If you chose to have EyeHealth Northwest bill your insurance on your behalf, you will be directed to log into our Secure Communications portal to complete our the entire ONLINE CONTACT LENS ORDERING INSURANCE BENEFITS and REGISTRATION FORM. ONCE WE HAVE VERIFIED YOUR INSURANCE BENEFITS, WE WILL CONTACT YOU VIA EMAIL TO CONFIRM YOUR BENEFITS AND TAKE YOUR ORDER.

If you have not been registered to use our online contact lens store and do not wish EyeHealth Northwest to bill your vision insurance, please click here and you will be directed to log into our Secure Communications portal to complete the Online Store Registration/Patient Information section of the form and submit the registration form to EyeHealth Northwest so we may complete your online contact lens ordering registration. We will notify you via secure email when you have been registered and you may place your order.
