Dr. Leak was born and raised in Alabama. He attended Auburn University where he received a B.S. in Animal Biology. In 2003, he graduated with honors from Southern College of Optometry in Memphis Tennessee. Immediately after graduation, Dr. Leak entered active duty service in the U.S. Army. Within one year, Dr. Leak was promoted to Chief of Optometry Services for Ft. Campbell, KY. Later, he joined the 101st Airborne Division as Chief of Optometry and served one year in Iraq where he was responsible for the eye care of about 50,000 soldiers. After leaving active duty in 2006, Dr. Leak entered private practice in Georgia. Over the years, Dr. Leak has gained valuable experience treating a variety of eye diseases including glaucoma, corneal disease, and ocular trauma. Dr. Leak has enjoyed volunteering his services with the Special Olympics of Georgia and being an active member of the Georgia Lions Club. He is also a member of the American Optometric Association, the Oregon Optometric Physician Association, and the Portland Metropolitan Optometric Society.
Dr. Leak moved to the Northwest early in 2009 with his wife, Melissa, and immediately fell in love with Oregon. In their spare time, he and his wife enjoy the beautiful outdoors of Oregon through hiking, kayaking, trips to the coast, and the Columbia River Gorge. And being from the South, Dr. Leak truly enjoys college football season every year.
Dr. Leak joined the staff of Eye Health Northwest in August 2009. He and his wife are active in their church and look forward to opportunities to give back and be involved in the community.

| PROFESSIONAL EXPERIENCE | Excel Eye Clinic Optometrist/Director of Store Operations Nov 2007 – Mar 2009 Provided comprehensive optometric care in a commercial setting. Was also given store management responsibilities that included staff management, management of daily operations of the store, writing and enforcing the Policies and Procedures Manual, and providing input into the overall direction of marketing and business strategy. Georgia National Guard Headquarters LaGrange Eye Clinic 101st Airborne (AASLT) Division Blanchfield Army Community Hospital Blanchfield Army Community Hospital Eyemasters Customer Service Supervisor/Trainer Aug 1997-May 1999 Started at entry level as a customer service representative in the in-bound call center. The call center supported a telecommunications company that provided cable, telephone, and internet services to over five cities in the Southeast. Was promoted to Supervisor within one year and given oversight on the daily performance of over thirty customer service representatives. My duties also included designing and implementing training on all services and software used by the company. Also provided consulting to branch offices on customer service improvement for their front desk personnel. |
