Dear Patients and Friends,
After 27 years of providing eye care at Eye Health Northwest, it is with mixed emotions that I am announcing my retirement at the end of 2018. I will continue to see patients through December 2018. It has been an honor and a privilege that you have trusted me with your eye care over the years. I am grateful for the many friendships I have had with you and your families. I will miss these the most.
Having myself just gone through the retirement of my long-term primary care provider, I am aware of how difficult the transition can be. With that in mind, I really want to make your transition to a new provider as smooth as possible. After extensive searching and interviewing, I am happy to announce Dr. Charlie Weber, MD has joined EyeHealth Northwest as my replacement. I could not be more pleased and recommend him highly. His training is impeccable and he is an overall wonderful person.

While I still very much enjoy the practice of Ophthalmology, I feel that the time is right for me and I am excited to see what the future holds. I hope to be able to continue serving as I have over the last 27 years in various ophthalmology clinics in developing countries, particularly in Latin America. I also have many hobbies that will keep me busy. I have been a beekeeper since age 12 and I am active with the OSU Master Beekeeper Program. I love woodworking, gardening, photography, biking, fishing and many other activities that I hope to continue.
Thank you for the years of confidence, friendship and trust. I wish you all the gift of good health. I am confident that you will be in good hands with the doctors at Eye Health Northwest.

Preparing for your Visit:

Prior to Your Appointment: Please complete and print the Patient history, HIPPA and payment policy forms even if you are an established patient. Once you have completed the forms, please bring them with you on the day of your appointment. Due to privacy issues, please do not attempt to send these forms to our office via e-mail.

Please check with your insurance company(s) to verify your “vision care” coverage for routine eye examinations by calling the customer service phone number listed on your insurance card. You may also call this same number to verify “medical” benefits for medically necessary visits and procedures.

The Day of Your Appointment:
Bring your current medical insurance card(s) with you.

  • The co-pay amount listed on your insurance card will be collected at the time of your visit.
  • For charges not covered by your individual insurance plan, payment is due at the time of service. Prompt pay discounts are available on certain services. Please ask at the front desk.

As part of your examination with us today, your pupils may be dilated. With dilation, most people experience light sensitivity and difficulty with near vision (such as reading). Some individuals also have difficulty seeing in the distance. The effects of the dilation can last several hours.
