In 1995, Dr. Cook joined the staff of EyeHealth Northwest. Dr. Cook practices general optometry and is an adjunct professor at Pacific University College of Optometry.
Rory M. Cook, OD was born and raised in Johannesburg, South Africa. He received his degree in microbiology and biochemistry at the University of Victoria in Victoria, British Columbia. He received his Doctorate of Optometry at Pacific University College of Optometry where he graduated"with distinction". While attending Pacific University, Dr. Cook was elected to Beta Sigma Kappa honor society. In 1995, Dr. Cook joined the staff of EyeHealth Northwest. Dr. Cook practices general optometry and is an adjunct professor at Pacific University College of Optometry.

Once you have completed the forms, please bring them with you on the day of your appointment. Due to privacy issues, please do not attempt to send these forms to our office via e-mail.

The Day of Your Appointment:
Bring your current medical insurance card(s) with you.
As part of your examination with us today, your pupils may be dilated. With dilation, most people experience light sensitivity and difficulty with near vision (such as reading). Some individuals also have difficulty seeing in the distance. The effects of the dilation can last several hours.
