Important Update for UnitedHealthcare Patients

Thank you for choosing Eye Health Northwest. We value your trust in us as your health care provider.
We would like to inform you of a change in our relationship with United Healthcare that impacts you as a member on a United Healthcare insurance plan. Eye Health Northwest is seeking to renew its agreement with United Healthcare because their payment does not cover our cost of providing treatment.
We are working with United Healthcare to find a solution that works for both parties. The agreement with United Healthcare may end on August 15th, 2026. After this date, your insurance may be out of network.
We are committed to keeping our patients informed and will update this page as soon as new information becomes available.
Current Insurance Participation
EyeHealth Northwest accepts most insurance plans and will bill both primary and secondary insurance plans as a courtesy. However, the primary responsibility for the account is yours.
If you are insured directly through the State of Oregon or the Oregon Health Plan, please bring your current medical card to your appointment.
Some insurance plans have restrictions or a specific provider panel that patients must access. Please check with your insurance carrier for specific eligibility, benefits, provider panel and/or referral requirements as they apply to you.
