Important Update for UnitedHealthcare Patients

Thank you for choosing Eye Health Northwest. We value your trust in us as your health care provider.
Unfortunately, we were not able to come to an agreement with United Healthcare that covers the cost of providing care. Our physicians, surgeons, providers, clinics and surgery centers are now out of network with United Healthcare. Our Vancouver clinic is still in network until 5/1/2027.
If you are a commercial patient, please contact your HR/Benefits Department and call United Healthcare to find an in-network provider.
If you are a Medicare Advantage patient, you may continue to receive care with our providers in our clinics only until the end of the year (12/31/2026). However, if you need surgery, our surgery centers are out of network. Medicare open enrollment for 2027 is from October 15th to December 7th, 2026; please check our insurance page at that time to see which Medicare Advantage plans we will be in network with for 2027.
Current Insurance Participation
EyeHealth Northwest accepts most insurance plans and will bill both primary and secondary insurance plans as a courtesy. However, the primary responsibility for the account is yours.
If you are insured directly through the State of Oregon or the Oregon Health Plan, please bring your current medical card to your appointment.
Some insurance plans have restrictions or a specific provider panel that patients must access. Please check with your insurance carrier for specific eligibility, benefits, provider panel and/or referral requirements as they apply to you.
